     For Breeders and Exhibitors
    

______	Nominate bitch after breeding and before whelping date
______	Download Bitch Nomination Form or nominate online
______	Complete all lines of Bitch Nomination Form
______	Mail Bitch Nomination Form with payment and postmark before whelping date
______	Receive bitch Futurity number within 4 weeks of nomination
______	Nominate individual puppies after birth and up to 4 months of age
______	Download Puppy Nomination Form or nominate online
______	Complete all lines of Puppy Nomination Form
______	Mail Puppy Nomination Form with payment and postmark on or before 4 months of age
______	Receive puppy Futurity number within 4 weeks of nomination
______	Notify Futurity Chairman of any name change of puppy for the Futurity nomination list sent to the show superintendent
______	Submit ABC Futurity entry and ABC class entry before show closing date
