Online Late Futurity/Maturity Puppy Nomination Form

I hereby nominate the following puppy/junior for the American Boxer Club Futurity. Payment of five ($5) dollar nomination fee along with the one hundred ($100) dollar late fee for a total of one hundred five ($105) dollars for said nomination (plus a $4.50 processing fee) to be paid via PayPal. Said nomination is subject to the "Conditions" governing the said Futurity. This nomination also makes the puppy/junior eligible for the following years Maturity. Said nomination is subject to the "Conditions" governing the said Maturity. The below submission represents that the information is correct and true.

The deadline for this late puppy nomination will be a postmark no later than 2 months before the first day of regular classes at the ABC National Specialty. Online nominations can only be accepted when accompanied by an online payment via PayPal Credit Card or PayPal eCheck. After recording the nominations the ABC Futurity/Maturity Puppy # will be returned to you via email as your receipt and acknowledgment. Any change of name submitted above must be conveyed to the Futurity/Maturity chairperson prior to the list being sent to the Superintendent (4 weeks prior to closing date of said show) or there is great risk that the entry will not be accepted for Futurity/Maturity. It is not necessary to notify Futurity chairperson of change of ownership of puppies.
